What to Expect at Your Appointment?
Please arrive 10-15 minutes before your scheduled appointment time so that you have time to check in and verify your information. All patients will have the option to check in ahead of time online. New patients must complete additional forms related to medical history. If you have not completed these forms online prior to your appointment, please arrive 20-30 minutes early.
When you arrive at your appointment, please have the following:
- A Photo ID
- A list of the medications, vitamins, and dietary supplements that you are taking now, or bring your medications with you for review
- Information about your past or present health issues, hospitalizations, family medical history, and immunizations
- Health insurance cards
- Co-payment for services
If you don’t have health insurance, please bring proof of income.
VitalPath Health accepts private pay.
New patients are able to complete their Patient Registration information before their appointment to save time. The Patient Registration packet will be sent to new patients by email or text message prior to your appointment. If you have not completed the packet, please arrive 20-30 minutes before your scheduled appointment time to complete these forms.
